A weak sheet might say “brakes checked” with a tick. A defensible sheet records the vehicle, time, driver, reported symptom, defect classification, action taken and release decision. The difference matters because the sheet records the operator's decision, not merely the existence of the truck.
The record must tell the whole story
The workshop then examines the braking system, records its findings and either rectifies the defect or documents why further monitoring is appropriate. If a repair is completed, the record should link the work to a job card or repair entry, identify the person who carried it out and show the subsequent re-check.

The entry identifies the exact wheel position rather than hiding the defect under “rear tyres”. The important point is not the number of words. It's the chain from observation to control. If the photographic reference were missing, the record would still have value, but the evidence would be weaker. The lighting section should cover the trailer as carefully as the tractor unit. Lamps may be serviceable at the lens while a connector, earth or wiring route is damaged. A form that asks only “lights working?” encourages a superficial answer.
Medium-risk groups such as coupling, load security and exhaust need clear prompts, while less frequently defective areas can use shorter but still identifiable lines. That arrangement can make the inspection quicker because it directs attention towards components with a stronger history of roadside intervention.
The wider trend isn't that maintenance has failed to improve. Logistics UK's 2025 compliance data says HGV annual-test initial failure rates fell from 32.4% in 2008/09 to 10.3% in 2024/25. Recording Defects and Driving Rectification
Every defect needs a short, controlled lifecycle:

A tyre with dangerous damage, an ineffective brake component or an insecure coupling requires a different response from a minor condition being monitored. The written procedure must make that distinction clear enough for drivers, transport staff and workshop personnel to apply it consistently.
A defect cannot be closed without a corrective action and named signatory. If a vehicle is moved only within a yard while awaiting repair, that restriction belongs on the record too. Otherwise, a later reviewer may reasonably assume the vehicle was released for normal road use.

The inspection file should be used before the vehicle reaches the test lane. Paper records are valid and inexpensive, but they depend on disciplined filing. A missing page, illegible signature or damaged sheet can break the sequence just when the operator needs to retrieve it.
A spreadsheet improves searchability and can track mileage, defect status and planned dates. It also creates its own risks. Different copies may circulate, users may overwrite entries and a defect can be marked complete without a linked repair or re-inspection.
A workshop management system can connect the inspection record with job cards, rectification, parts ordering, brake tests and MOT preparation. That can produce one retrievable history per vehicle or trailer, but only if the staff enter complete information rather than treating the system as a faster tick box.

The medium is not what makes the record defensible. Completeness, traceability and retrieval do. The practical choice is the system the least technically confident operator can use consistently, with clear ownership for reviewing open defects and overdue inspections.
